Peyronie's Disease Treatment in Dubai

What is Peyronie’s Disease?

Peyronie’s disease occurs when fibrous scar tissue forms in the deeper layers beneath the penile skin, resulting in curvature, reduced size, or deformity.

This scar tissue may cause pain, and when an erection occurs, the penis may curve up, down, or to the side, depending on where the plaque is located.

In some cases, the penis may appear hourglass-shaped or indented. Peyronie’s disease can lead to erectile dysfunction (difficulty getting or maintaining an erection) and can make sexual activity painful or difficult.

While a slight curve in the penis is familiar, Peyronie’s disease causes a significant bend or pain. The condition rarely improves on its own and may worsen without peyronie’s disease treatment. Early intervention may help prevent it from getting worse and reduce symptoms.

Peyronie’s Disease Symptoms

Peyronie’s Disease symptoms include:

  • Scar tissue (plaque): Felt as lumps or hard tissue under the skin of the penis.
  • Penis bend: Curving upward, downward, or to one side.
  • Erection issues: Difficulty getting or maintaining an erection (erectile dysfunction).
  • Penis shortening: Decrease in length during erections.
  • Pain: Pain in the penis, with or without an erection.
  • Changes in appearance: Narrowing, indentations, or hourglass shape of the erect penis.
  • Pain during sex: Pain or discomfort experienced during sexual intercourse (dyspareunia).
  • Softer erections: Less firm erections than usual.
  • Lumps: Scar tissue lumps on the penis.

Pain typically improves within 1-2 years, but curvature and shortening may remain.

Peyronie’s Disease vs. Normal Curvature

It is essential to distinguish between Peyronie’s Disease and the natural curvature that some men have. Some degree of curvature is standard and does not typically cause problems. However, Peyronie’s disease is marked by an abnormal bend that often leads to pain and difficulty with sexual activity.

Dr. Muthana Alrawi provides expert care to help manage and treat Peyronie’s Disease, offering personalized Peyronie’s Disease Treatment in Dubai based on each patient’s unique needs and medical history. The treatment approach varies depending on the severity of the condition and its progression.

Traction Therapy

Traction therapy uses external devices, like a vacuum erection device or traction device (e.g., RestoreX®), to gently stretch the penis and sometimes reverse the curve. This method helps encourage the scar tissue to break down. Benefits include:

Improved penile curvature.

Restoration of lost length.

Increased erection hardness.

Duration: Most users must use the device for several months to see improvements.

Side Effects: There are typically no long-term side effects when used correctly.

Medications

Healthcare providers may recommend medications if you’re in the early stages of Peyronie’s disease or unsure about surgery. Medications are generally more effective at managing symptoms, but they may not result in a wholly straightened penis.

Injection Medications: Collagenase injections (Xiaflex®): Recommended for curves greater than 30 degrees to aid in breaking down scar tissue. Typically, two injections per week for eight weeks.

Interferon injections: A protein that reduces swelling and scarring by releasing an enzyme that dissolves scar tissue.

Verapamil injections (Isoptin®): Originally for high blood pressure, verapamil may help reduce pain and curvature.

Oral Medications:

Tadalafil (Cialis®): Improves blood flow, potentially preventing worsening of scarring and helping with erectile rigidity.

Pentoxifylline (Pentopak®): Improves blood flow to the penis.

L-arginine: An essential amino acid that promotes better blood flow.

Colchicine (Colcrys®): Reduces inflammation by lowering uric acid and helps reduce penile swelling.

Note: No oral medications have been shown to reduce curvature or plaques reliably, but they may help alleviate symptoms.

Potassium aminobenzoate (Potaba®): A form of vitamin B that may reduce the size of scars but not curvature.

Note: No oral medications have been shown to reduce curvature or plaques reliably, but they may help alleviate symptoms.

Surgery

Surgery is generally advised only for severe cases of Peyronie’s disease, particularly when it significantly interferes with sexual activity. Surgery is considered in the stable or chronic phase once scarring and curvature have stopped progressing and after at least 6-12 months without pain.

Surgical Options:

Shortening the unaffected side of the penis: Involves making minor adjustments to the unaffected side to straighten the penis. This may be viable if you can still achieve strong erections and haven’t lost much length. However, risks include length loss, reduced sensitivity, and erectile dysfunction.

Lengthening the inside of the curve: If the curve is severe or the shaft is very short/narrow, the surgeon may remove or cut scar tissue and cover the space with material. This approach is considered for those with severe curvature and/or loss of length but can come with risks to erectile function and sensitivity.

Penile Implant: Recommended for those with significant curvature and erectile dysfunction who don’t respond to oral treatments. The implant allows for a straight, rigid erection without the need for medications, which can last as long as desired. The implant can be deflated when no erection is desired.
